SPONSOR:
This Sweepstakes (“Sweepstakes”) is sponsored by UBI Telehealth (“Sponsor”). The Sponsor’s address is 11215 Metro Parkway, Building 3, Suite 1, Ft. Myers, FL 33966.
SWEEPSTAKES ENTRY PERIOD:
The Sweepstakes entry period begins at 12:01 AM Eastern Time (ET) on November 13, 2024, and ends at 11:59 PM ET on December 5, 2024 (“Entry Period”).
HOW TO ENTER:
To enter, visit the official Sweepstakes registration page located at http://ubitelehealth.com/sweepstakes-live-fest-2024 and follow the on-screen instructions to complete and submit the entry form.

Limit one (1) entry per person.
Entries may be made by email or mobile phone number during the Entry Period.
Entries received after the end of the Entry Period or by any other means not specified in these Official Rules will not be eligible.
No third-party entries or entries submitted through sweepstakes services are permitted.

ELIGIBILITY:
This Sweepstakes is open to legal residents of the United States who are 18 years of age or older at the time of entry. Employees of the Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members (spouses, parents, children, siblings) or household members are not eligible to participate. Void outside the United States and where prohibited by law.
GRAND PRIZE:
One (1) grand prize will be awarded, consisting of:

WINNER SELECTION AND NOTIFICATION:
On or about December 6, 2024, a random drawing will be conducted from among all eligible entries received during the Entry Period. The potential winner will be notified by email using the email address provided on their entry form. If the potential winner does not respond within three (3) calendar days after the notification, or if the prize notification is returned as undeliverable, the prize will be forfeited, and an alternate winner may be selected.
CLAIMING THE PRIZE:
The winner must accept the prize “as is.” The winner will be required to provide valid identification to verify eligibility and may be required to sign and return an Affidavit of Eligibility, a Liability Release, and a Publicity Release (where legal) within three (3) business days of notification. If the winner fails to comply with these requirements, the prize may be forfeited and awarded to an alternate winner.
GENERAL CONDITIONS:

By participating in the Sweepstakes, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all matters related to this Sweepstakes.
The Sponsor is not responsible for any incorrect or inaccurate information, or for any technical failures, including any malfunction or failure of the entry process or any interruption or delay in the operation of the Sweepstakes website, whether caused by technical problems or otherwise.
By accepting the prize, the winner agrees to release, discharge, and hold harmless the Sponsor, its affiliates, and their respective directors, officers, employees, and agents from any liability, injury, loss, or damage of any kind resulting from the acceptance, use, or misuse of the prize.
The winner may be required to provide a social security number or tax identification number to the Sponsor for tax purposes.

PRIVACY:
Information provided by entrants will be used solely for the purpose of this Sweepstakes and future marketing efforts by the Sponsor, and will be handled in accordance with the Sponsor’s privacy policy.
LIMITATION OF LIABILITY:
By entering this Sweepstakes, entrants agree to release and hold harmless the Sponsor and its affiliates from any and all liability for any injuries, losses, or damages of any kind caused by participation in this Sweepstakes or acceptance and/or use of the prize.
DISQUALIFICATION:
The Sponsor reserves the right, in its sole discretion, to disqualify any individual found to be tampering with the entry process, the operation of the Sweepstakes, or violating these Official Rules.
SWEEPSTAKES VOID WHERE PROHIBITED:
This Sweepstakes is void where prohibited or restricted by law.

SPONSOR’S DECISIONS ARE FINAL:
All decisions of the Sponsor regarding this Sweepstakes are final and binding in all respects.
GOVERNING LAW:
The Sweepstakes is governed by the laws of the State of Florida, without regard to its conflicts of law principles.
